事件二,监理单位要求项目部针对现场实际施工编制关键工序的作业卡片和安全预防措施方案,上报监理单位;
事件三,现场监理程师在对现场进行检查时,发现正在填筑的路基坡脚出现隆起,立即上报了现场的,经过现场察看监理组长立即停止现场的填筑施工,安排施工单位做好警示标志,防止路基垮塌;通过检查发现路堤中心沉降为12mm/d,边桩的水平位移7mm/d;塑料排水板长度仅为7m。通过对内业资料发现无过程中自检资料,最终的质检资料却显示合格;

事件五:项目部编制的关键工序作业卡片明确基床底层按照“四区段、六流程”填筑施工,填料分层的压实厚度按照35cm进行控制。
【背景资料】
某施工单位承接了一座多跨变截面预应力混凝土连续箱梁桥,大桥分为上下游两幅,每幅单箱顶板宽10.5m,底板宽6m。大桥采用钻孔灌注桩基础,双柱式桥墩(墩柱高15m至26m不等),普通钢筋混凝土盖梁。
上部结构0号块采用墩顶混凝土现浇施工,临时固结构造示意图如下图。

其他梁段(1~19号)采用预制场长线法台座预制,缆索吊装系统悬臂拼装。施工单位根据构件预制场的布设要求,立即进行了箱梁预制场的选址和规划,并编制了《梁场布置方案》,在报经企业技术负责人审批后实施。
各梁段之间腹板采用剪力齿衔接,环氧树脂粘合,顶板与底板均设20cm湿接缝。施工中加强测量管理,各梁段施工按照设计标高安装定位,控制好全桥线形。
1~19号梁段长线法预制及悬拼安装施工工序为:预制场及存梁区布置→梁段浇筑台座准备→梁段浇筑及养护→D→梁段外运→梁段吊拼就位→临时预应力张拉及腹板剪力齿粘合→E→预应力穿索与张拉、封锚→下一梁段施工。
按照交通运输部颁布的《公路桥梁和隧道工程施工安全风险评估指南(试行)》的要求,施工单位对全桥进行了总体风险评估,评估结果为Ⅲ级。
某施工单位承建了一段山区高速公路,其中有一座21×40m先简支后连续T型预应力混凝土梁桥,北岸桥头距隧道出口30m,南岸桥头连接浅挖方路堑,挖方段长约2km。大桥采用双柱式圆形截面实心墩,墩身高度10~40m,大桥立面布置示意图如图3所示。
