背景资料：

某城市新建下穿隧道，左线长 3316m，右线长 3200m，隧道最小埋深 20m，净空宽度 9.64m，净空高度 6.88m， 净空面积为 58.16m2，设计车速 100km/h，开工日期为 2016 年 7 月，2017 年 7 月竣工，上部为既有道路，周边建筑物较多。

该段隧道复合式衬砌横断面示意图如图所示，采用喷锚网联合支护形式，结合超前小导管作为超前支护措   施，二次衬砌采用补偿收缩混凝土，初期支护与二次衬砌之间铺设防水层。在一个模筑段长度内灌注边墙混凝   土时，施工单位为施工方便，先灌注完左侧边墙混凝土，再灌注右侧边墙混凝土。2017 年 6 月 6 日上午，隧道开挖时，量测人员在处理量测数据中，发现“周边位移—时间曲线”出现反弯点，但未及时告知作业班组潜   在的危险，当日下午发生较大塌方，当场死亡 5 人，重伤 12 人。经补报并核实，截止 2017 年 7 月 4 日，确认累计死亡人数达 10 人。事故发生后，施工单位加强了监控量测。
