【背景资料】某施工单位承建客运专线某黄土隧道工程。该工程为单洞双线隧道，全长4300m，起止里程为DK126+000～DK130+300。隧道围岩级别以IV、V级为主，DK127+500～DK127+900段局部有地下水。全隧按进、出口两个工区组织施工。该隧道V级围岩地段采用开挖方法示意图所示的开挖方法施工，施工工艺详见施工工艺流程图。施工中发生以下事件：4月2日上午7时，施工人员开始对DK127+512～DK127+522段仰拱进行施工。
施工内容包括：挖掘机开挖、基坑抽水、补打边墙锁脚锚杆、钢拱架架设、喷射混凝土等作业。当日下午4时在喷射混凝土作业时，安全员发现拱部初期支护混凝土开裂、掉块，钢拱架变形，立即通知现场人员撤离。10分钟后该段发生坍塌，未造成人员伤亡。
背景资料
某建设单位投资兴建一办公楼，建筑面积21000㎡ ；钢筋混凝土框架剪力墙结构。
施工过程中，监理单位下发针对地下室地面做法的设计变更指令，在变更指令下发后第15天，施工单位向监理工程师提出该项变更的估价申请。监理工程师审核时发现计算有误，要求施工单位修改。于变更指令下发后的第19天，监理工程师再次收到变更估价申请，经审核无误后提交建设单位，但一直未收到建设单位的审批意见。次月底，施工单位在上报已完工程进度款支付申请时，包含了经监理工程师审核、已完成的该项变更所对应的费用，建设单位以未审批同意为由予以扣除，并提出变更设计增加款项只能在竣工结算前最后一期的进度款中支付。施工单位对此提出异议。
案例四
【背景资料】
A机电安装公司承接了某钢厂中板生产线安装工程，合同签订时工程内容包括推钢式加热炉筋管耐热滑块的焊接安装。11t的加热炉气包吊装，吊装高度24m，16台行车吊装，轧机安装，单件最大重量102t；高压水系统，液压润滑系统安装(包括管道的化学处理）。
经业主同意，A公司将加热炉气包吊装分包给了专业吊装公司B公司。B公司在A公司安全管理制度的基础上编制了应急预案，并进行定期和不定期的安全检查。
因工期紧张，又逢雨季，项目部针对施工具体情况制定了相应的安全技术措施。并严格执行施工前的安全技术交底制度。
由于工期紧，交叉施工多，深基坑多，发生了下列事件：
事件一，B公司在轧机102t最大件吊装时，吊索具2t，B公司拟采用一台起重能力为110t的大型吊车起吊，但遭到监理公司的制止。
事件二，由于深基坑无护栏和标识，7月22日上午10时，两名施工人员不慎落入，造成一人重伤，一人死亡。7月23日下午，施工项目部安全部门负责人向A公司上报了事故，A公司负责人接上报告后，考虑到已是周末，于是等到7月26日（周一）上午报告了当地人民政府安全生产监督管理部门。
案例（二）背景资料
某施工单位承建位于西北地区的某新建高速铁路站前工程第3标段。该标段路基工程有两段黄土路堑和一段软土路堤。黄土路堑段长度分别为80m和90m，路堑开挖最大深度6m，边坡防护采用混凝土拱形骨架护坡。软土路堤段长度200m，填筑最大高度4m，地基采用CFG桩加固，路堤填筑完成后采用堆载预压。
施工过程中发生以下事件：
事件一：施工单位进场后，测量主管工程师制定的线路复测技术方案主要内容包括：复测内容有中线测量、中平测量和横断面测量；当复测与定测成果不符值在规范规定的限差范围内，采用复测成果；复测完成后进行成果分析，形成复测报告。项目经理部总工程师审核时认为复测技术方案存在不妥之处，要求进行修改。
事件二：黄土路堑正式开工前，项目经理部给监理单位报送了开工申请报告及相关资料。计划在旱季进行黄土路堑土方开挖和边坡防护施工，工期4个月；土方开挖施工前做好堑顶截、排水和地面排水沟；土方开挖过程中采取有效措施防止地表水流入开挖施工区域；待两段路堑土方开挖全部完成后，统一进行混凝土拱形骨架护坡施工。监理工程师认为项目经理部的施工组织方案不妥，未批复开工报告。
事件三：CFG桩施工前，项目经理部按规范要求进行了工艺性试验。按设计要求，桩帽采用现浇法施工，工艺流程如图2所示。
事件四：软土路堤填筑完成后堆载预压施工前，项目经理部编制的技术交底内容包括：堆载预压材料应符合设计要求，不得使用淤泥土或含垃圾杂物的填料；堆载施工过程应严格控制加载速率，按规定进行路基沉降变形观测，并做好观测记录；堆载预压时间计划180天，项目经理部进行卸载评估后，经监理单位验收通过方可卸载。

The casino included a photo of Williams among those of banned gamblers, and wrote to him a” cease admissions” letter noting the medical/psychological nature of problem gambling behaviors, the letter said that before being readmitted to the patronizing the casino would pose no threat to his safety have to his safety or well-being.
